About the Role:
As a Care Coordinator , you will be responsible for managing day-to-day care staffing for clients in both our In-Home (Kansas) and HCY (Missouri) programs. You’ll work closely with families, caregivers, and clinical teams to ensure quality care, timely communication, and continuity of services.

What You'll Do:

• Coordinate caregiver schedules based on client needs and caregiver qualifications
• Build meaningful relationships with families and caregivers to ensure trust and care satisfaction
• Maintain accurate documentation and reports per program guidelines
• Communicate clearly and professionally across departments and with external partners
• Assist with onboarding and training new caregivers to ensure a high standard of care
• Respond to staffing needs quickly and creatively to ensure client support
• Represent Phoenix with professionalism and empathy at all times

What You Bring:

• A true passion for working with children with special needs and the elderly
• Experience in home care, case coordination, scheduling, or human services preferred
• High school diploma or equivalent required
• Ability to multitask, problem-solve, and adapt quickly in a fast-paced environment
• Strong communication and organizational skills
