About the department: The goal of the Home Health Department is to decrease the number of hospital days and hospital visits per patient by providing skilled nursing and other services in the patient's home. Also, to instruct and assist patients and their families regarding their diagnosis, new procedures, medications, prevention of complications, control of pain, and general home care. Individualized plans of care are developed and used to assess, intervene, and treat home-bound patients who may not otherwise receive the healthcare they need due to physical, mental, or socio-economic circumstances. A typical day includes: Traveling to the homes of patients to assess conditions and treatment plan progress, and providing therapies aimed at improving mobility and recovery.
